Information to Have Ready
Medical and Dental History
Prepare a current medication list and details of relevant health conditions, allergies and previous dental treatment. Note any new symptoms or concerns you would like to discuss.
Coverage Information
If you use dental benefits or the Canadian Dental Care Plan, have your current information available. The team can explain what is needed for your appointment enquiry.
Previous Dental Records
If records are needed from another office, ask about the authorization and transfer process. Providing the previous clinic's contact information is a useful starting point.
Get Help with Your Forms
If you cannot complete paperwork in advance, contact us before your visit. Ask what help is available and how much time to allow when you arrive.
